Project
The United Arc
of Franklin and Hampshire Counties
This program
offers support to teens and young adults that
are transitioning from foster care or Department
of Social Services’ (DSS)
supervision to independent adulthood. Services
include:
Personal Futures
Planning – Staff
use a variety of person-centered planning
tools to help young people envision their
futures and identify the steps for turning
dreams into realities.
Case Management – Coordination of formal
and informal services with local and state
agencies. Guidance through the
application process for programs offered
through the Dept. of Mental Retardation,
Dept. of Mental Health, Massachusetts
Rehabilitation Commission, Dept. of Transitional
Assistance, Social Security, local and
regional housing authorities, community
action programs, and so forth.
Assistance with
Essential Life Skills – To
help each individual find or maintain
an appropriate living situation, and manage
household tasks, personal finances, hygiene
and appearance, achieve positive habits
for maintaining good health through appropriate
exercise and nutrition, social skills development,
community connections, decision-making
and pursuit of educational goals.
Job Placement,
Training & Support – Assistance
with skills assessment and career interest
inventory, resume preparation, job search,
assistance in completing applications
forms ad preparing for interviews, liaison
with employers and/or assistance with application
to other job assistance programs.
Support & Training Groups – Opportunities
to meet with other youth and young adults
for time limited group support for social
skill development,, abuse prevention, job
readiness, self-advocacy, or preparation
for parenthood.
Eligibility & Referral –Transition
services are provided to teens and young adults
referred through the Department of Social Services.
